Digital registration form

We would like to inform you about our registration policy at Medi-Mere. Here are some important points:

  • If you move from outside Almere to an address within the postcodes listed below, you can register with us.
  • If you move within Almere to an address within the postcodes listed below, you can register with us.
  • If you move within Almere within the same postcode area, you cannot register with us.
  • If you are not moving and are already registered with another general practitioner, you cannot register with us.

 

Attention:

Due to the opening of registrations for our new location, Medi-Mere Hout, it may take longer to process your registration.

 

Until your registration is processed and our medical assistant has reviewed it with you, you will remain a patient at your current health center. We rely on your current general practitioner to process your registration, as we request your medical records from them.

Our medical assistant will contact you as soon as possible. Thank you for your patience and understanding.

 

Are you already registered with Medi-Mere and living in the Hout district? We will contact you about the transition by September 16. You can also email us at communicatie@medi-mere.com for any questions.

 

For new registrations, we adhere to the following postcode areas; outside these postcodes, unfortunately, we cannot register you:

  • Medi-Mere Stad: 1311 t/m 1316, 1319, 1321 t/m 1324
  • Medi-Mere Poort: 1309,1361, 1362, 1363
  • Medi-Mere Hout: 1341, 1343, 1344, 1345, 1346, 1347, 1348, 1349, 1358
  • Medi-Mere Haven: 1351 t/m 1359
  • Medi-Mere Buiten: 1331 t/m 1336, 1338, 1339, 1341, 1343 en 1349

 

For residents of the Hout district, within the mentioned postal codes, we make an exception, and they are always allowed to register.
